Chest Pain: Causes, Warning Signs, Diagnosis and When to Seek Emergency Care
What is chest pain, and why does it matter?
Chest pain is any discomfort, pressure, tightness, burning, or ache felt anywhere between the neck and the upper abdomen. It is one of the most common reasons people seek emergency care, and for good reason: while most chest pain has a benign cause, a smaller proportion is caused by conditions such as heart attack, pulmonary embolism, or aortic dissection that can be fatal without prompt treatment.
This guide is written for anyone experiencing chest pain themselves, or supporting a family member who is. It explains how doctors think about chest pain, what the most common causes are, which features suggest a benign cause versus a dangerous one, what tests are typically used to investigate it, and — most importantly — when you should seek urgent or emergency medical care rather than trying to "wait it out".
Common misconceptions about chest pain
- "Heart attacks always cause severe, crushing pain." Some heart attacks — particularly in women, older adults, and people with diabetes — present with milder discomfort, breathlessness, fatigue, or nausea rather than classic crushing pain.
- "If I can press on the area and it hurts, it can't be my heart." Reproducible pain on pressing (tenderness) does make a musculoskeletal cause more likely, but it does not fully rule out a cardiac cause, especially in people with risk factors.
- "Young, fit people don't get heart attacks." It is less common, but coronary artery disease, spontaneous coronary artery dissection, and inherited heart conditions can affect younger and physically fit people.
- "Chest pain that comes and goes isn't serious." Intermittent chest pain — particularly with exertion — can be a symptom of angina and should still be assessed.
If you are ever in doubt about whether your chest pain could be serious, treat it as an emergency. It is always appropriate to seek urgent assessment for chest pain — doctors and paramedics expect and welcome this.
When to seek medical care
Call emergency services (999/911) immediately if you have:
- Central crushing, squeezing, or heavy chest pain, especially spreading to the jaw, neck, back, or left arm
- Chest pain with sweating, nausea, vomiting, or breathlessness
- Sudden, severe, tearing chest or back pain (possible aortic dissection)
- Sudden sharp chest pain with breathlessness, especially after long travel, surgery, or immobility (possible pulmonary embolism)
- Chest pain with fainting, severe dizziness, or a very fast/irregular heartbeat
- Chest pain with blue or grey lips, confusion, or extreme weakness
Seek urgent medical assessment (same day) if you have:
- New chest pain with exertion that eases with rest (possible angina)
- Chest pain with a fever and productive cough
- Chest pain that is new, unexplained, and persistent for more than a few minutes
- Chest pain in someone with known heart disease, diabetes, or strong cardiac risk factors
Seek routine medical advice if you have:
- Mild, reproducible pain that worsens when pressing on the chest wall
- Occasional burning discomfort clearly linked to meals or lying flat
- Chest tightness that consistently occurs alongside anxiety or panic symptoms
Cardiac causes of chest pain
- Angina — chest tightness triggered by exertion or stress, caused by reduced blood flow to the heart muscle, usually easing within minutes of rest.
- Heart attack (myocardial infarction) — sudden blockage of a coronary artery causing damage to heart muscle; pain is often severe, persistent, and accompanied by sweating, nausea, or breathlessness.
- Pericarditis — inflammation of the sac around the heart, often causing sharp pain that improves when leaning forward.
- Arrhythmia — abnormal heart rhythms can cause chest discomfort alongside palpitations or dizziness.

Common non-cardiac causes
- Musculoskeletal pain — costochondritis (inflammation of the rib cartilage) or muscle strain; typically reproducible by pressing on the chest wall or moving the torso.
- Acid reflux (GORD) — a burning sensation behind the breastbone, often after meals, spicy food, or lying flat.
- Pleuritic pain — sharp pain that worsens with deep breathing or coughing, often following a chest infection.
- Anxiety and panic attacks — chest tightness with breathlessness, tingling in the hands, and a sense of impending doom; a real physical experience that still warrants assessment if new.
- Shingles — a band-like burning pain that may precede a rash on one side of the chest.
How doctors investigate chest pain
The initial assessment usually involves a focused history (what the pain feels like, how it started, and any associated symptoms), a physical examination, and some or all of the following tests:
- ECG (electrocardiogram) — records the heart's electrical activity to look for signs of a heart attack or arrhythmia.
- Troponin blood test — measures a protein released when heart muscle is damaged; often repeated a few hours apart.

Treatment and management
Treatment depends entirely on the underlying cause. Cardiac causes may require medications (such as aspirin, nitrates, or statins), procedures such as angioplasty, or urgent hospital admission. Musculoskeletal pain is often managed with rest, gentle stretching, and simple pain relief. Reflux-related pain may respond to dietary changes and acid-suppressing medication. Anxiety-related chest tightness can improve with breathing techniques, psychological support, and treatment of the underlying anxiety disorder — but should still be properly assessed the first time it occurs.
Recovery, follow-up and prevention
Recovery timelines vary widely: musculoskeletal pain often settles within days to a few weeks, while recovery from a cardiac event may involve weeks of cardiac rehabilitation and long-term medication. Reducing future risk generally involves managing blood pressure, cholesterol, and blood sugar, stopping smoking, staying physically active, and attending recommended follow-up appointments.
